Inpatient Rehabilitation

South Eastern Private Hospital welcomes patients from all private health funds DVA, TAC, Workers Compensation and those who are self funded.

Our rehabilitation program caters for a wide and diverse patient population. Patients are admitted under the care of a specialist doctor and receive treatment from a multidisciplinary therapy team. The multidisciplinary team meets regularly to assess and formulate patient care pathways, treatment plans, progress and discharge plans.

Complete admission paperwork

If you have a planned admission, you will receive admission paperwork from your Doctor's rooms. Please ensure you complete the forms and return them to South Eastern Private Hospital.

Where possible, we require your admission forms one week prior to your admission date. This is important so that we can confirm your hospital booking.

For rehabilitation patients transferring from another facility, where possible you will be assessed by our Rehabilitation Assessor to ensure suitability to programs provided by the hospital.  A number of information brochures about Brunswick Private, our facilities and programs will also be provided at this time.

Confirm costs and insurance coverage

Prior to your admission, please contact our admissions staff to verify your insurance and any out-of-pocket expenses you may incur from your hospital stay. Our staff are happy to answer any questions you may have about costs and insurance coverage. You may also wish to check with your health fund if you have any concerns.

Admission confirmation

It is important that you contact our Admissions staff by calling 03 9549 6555 the day before your scheduled admission to confirm your admission details.

What to bring to hospital

Please remember to bring the following to hospital (if relevant to you):

  • Letter of referral from your treating Doctor or Specialist, unless this has already been sent to the hospital
  • Medicare card
  • Private health insurance card
  • Pensioner card
  • Veterans' Affairs entitlement card
  • Drivers licence or accepted identification
  • Workers Compensation or Transport Accident Commission insurance details
  • Relevant X rays
  • Health care cards and pharmacy safety net details
  • All medications you are currently taking (in original packaging) and any repeat prescriptions
  • Blood group card (if available)
  • Health history and current medication
  • Sleepwear, dressing gown and slippers (for inpatients)
  • Comfortable footwear
  • Personal toiletries
  • Glasses and physical aids (e.g. walking stick, hearing aid)
  • Sufficient cash for incidental expenses (no more than $50)

Do not bring the following items with you:
 

  • Talcum powder
  • Jewellery
  • Large sums of money
  • Other valuables

For all electrical appliances (if essential), please advise hospital staff on arrival as they must be tested by the hospital Maintenance Department for safety reasons.

On arrival

On the day of your admission, ensure you arrive to hospital at the time designated by our admissions staff or your Doctor. On arrival, please register at reception. You will then be shown to your room.

Medication

On admission to hospital, you’ll be asked about the medications you are currently taking, and whether you have had any unexpected reactions or allergies to medications in the past. If so, these will be noted on your medication chart.
